Location & Overview

Extensor carpi radialis longus is a superficial muscle located on the radial (thumb) side of the posterior compartment of the forearm.[1,2] It belongs to the superficial layer of the extensor group, alongside muscles such as brachioradialis, extensor carpi radialis brevis, extensor digitorum, extensor digiti minimi, extensor carpi ulnaris, and anconeus.[1,2] It is a fusiform muscle, meaning it has a spindle-like shape that tapers at each end.[2]

Proximally, brachioradialis partly overlaps extensor carpi radialis longus, and the two muscles often blend together at their borders.[1,2] The radial nerve passes through the region between brachialis on the medial side and brachioradialis together with extensor carpi radialis longus on the lateral side at the level of the elbow.[1]

As the muscle transitions into its tendon in the distal forearm, the tendon is crossed superficially by abductor pollicis longus and extensor pollicis brevis.[1,2] Near its insertion, extensor pollicis longus also crosses the tendon obliquely.[1] The distal tendon of extensor carpi radialis longus accompanies extensor carpi radialis brevis beneath the extensor retinaculum, where both tendons travel together within a common sheath.[1,2] The primary role of extensor carpi radialis longus is to extend and abduct the wrist, and it also contributes to stabilizing the wrist during forceful gripping.[1,2]

Origin & Insertion

Extensor carpi radialis longus originates from the distal third of the lateral supracondylar ridge of the humerus.[1,2] Additional fibers arise from the front of the lateral intermuscular septum, and some fibers may also join from the common extensor origin.[1] The lateral supracondylar ridge is the bony ridge running along the lower outer portion of the humerus, just above the lateral epicondyle, and it serves as an attachment for several forearm muscles in this region.

The muscle inserts onto the radial side of the dorsal surface of the base of the 2nd metacarpal.[1,2] This means the tendon attaches to the thumb-side edge of the back of the bone at the base of the index finger. As a normal variant, slips from the tendon may occasionally extend to the base of the 1st or 3rd metacarpal, and the tendon may also contribute to the intermetacarpal ligaments.[1]

Action & Function

Extensor carpi radialis longus has two primary actions at the wrist. It extends the wrist, which involves bending the hand backward (raising the back of the hand toward the forearm). It also performs wrist abduction, also known as radial deviation, which tilts the hand toward the thumb side.[1,2] These actions occur at both the wrist and midcarpal joints.[1]

Wrist abduction (radial deviation): Extensor carpi radialis longus is also a strong wrist abductor. During wrist abduction, the hand tilts laterally toward the radius and the thumb.[1,2]

Wrist stabilization during fist clenching: Extensor carpi radialis longus is active during fist clenching and plays an important role in stabilizing the wrist when the fingers are tightly flexed.[2] When the long finger flexor muscles contract to close the hand, they would also tend to flex the wrist. Extensor carpi radialis longus, working together with extensor carpi radialis brevis and extensor carpi ulnaris, counteracts this pull and holds the wrist in a stable, slightly extended position. This allows the finger flexors to generate their full gripping force without the wrist collapsing into flexion.[2]

Innervation

Extensor carpi radialis longus is innervated by the radial nerve, with nerve root values of C6 and C7.[1,2] The motor branch to extensor carpi radialis longus arises from the radial nerve proximal to the elbow, in the region where the radial nerve passes between brachialis and the brachioradialis/extensor carpi radialis longus group.[1]

Although the named nerve supply is consistent, the branching pattern can vary between individuals. Bertelli et al. (2024) found in a fresh-cadaver study of 34 upper limbs that extensor carpi radialis longus may receive one, two, or three separate motor branches from the radial nerve.[3] This variation in branching is relevant for surgical planning but does not change the standard gross-anatomy description of the nerve supply.

Blood Supply

The principal arterial supply to extensor carpi radialis longus comes from a branch of the radial recurrent artery.[1] The radial recurrent artery is a branch of the radial artery that courses proximally to supply structures around the lateral aspect of the elbow. Additional blood supply is provided by the radial collateral branch of the profunda brachii artery and, more distally, by the radial artery itself.[1] Together, these vessels supply the muscle along its length from the proximal arm region to the distal forearm.
