Location & Overview
The extensor carpi radialis brevis is a superficial extensor muscle on the radial (thumb) side of the forearm.[1,2] It belongs to the superficial layer of the posterior extensor-supinator group and is also considered part of a proximolateral cluster known as the mobile wad, together with the brachioradialis and extensor carpi radialis longus.[1] The official anatomical name for the muscle is musculus extensor carpi radialis brevis.[3]
Extensor carpi radialis brevis is shorter than its close neighbor, extensor carpi radialis longus, and is covered by it in the proximal forearm.[1,2] Further distally, the tendon of extensor carpi radialis brevis passes deep to the tendons of abductor pollicis longus and extensor pollicis brevis.[1] The tendons of extensor carpi radialis brevis and extensor carpi radialis longus then travel together beneath the extensor retinaculum within a shared synovial sheath in the second dorsal compartment of the wrist.[1,2] Within this compartment, extensor carpi radialis brevis lies medial to extensor carpi radialis longus in the dorsal groove of the radius.[1]




Origin & Insertion
Extensor carpi radialis brevis arises from the lateral epicondyle of the humerus through the common extensor tendon, an origin it shares with extensor digitorum, extensor digiti minimi, and extensor carpi ulnaris.[1,2] Additional fibers of origin attach to the radial collateral ligament, a strong covering aponeurosis, and adjacent intermuscular septa.[1] The posterolateral ligament of the elbow blends with the bony attachments of the supinator and extensor carpi radialis brevis in this area, reflecting the density of overlapping structures at the lateral epicondyle.[1] Greenbaum et al. (1999) found that at the osseotendinous junction there may be no clear separation between the extensor carpi radialis brevis and extensor digitorum origins, reinforcing the practical concept of a shared common extensor origin.[4]
The muscle inserts onto the dorsal base of the third metacarpal.[1,2] More specifically, the insertion reaches the radial side of the dorsal base of the third metacarpal and may extend to adjoining parts of the base of the second metacarpal.[1] Variable distal slips between the second and third metacarpals have been recorded.[1] A cadaveric variation study by Nayak et al. (2008) also demonstrated examples of extensor carpi radialis brevis inserting into the bases of both the second and third metacarpals, supporting the broader range of distal attachment that has been described.[5]


Action & Function
The primary action of extensor carpi radialis brevis is wrist extension, meaning it straightens the wrist by pulling the dorsal (back) surface of the hand upward.[1,2] Because its tendon inserts centrally at the third metacarpal base rather than on the ulnar side of the hand, extensor carpi radialis brevis is considered a particularly efficient wrist extensor.[1]
Wrist abduction: Extensor carpi radialis brevis also contributes to wrist abduction, also known as radial deviation, which is the movement of tilting the hand toward the thumb side.[1,2] It performs this action together with extensor carpi radialis longus, and when the radial wrist extensors work alongside flexor carpi radialis, they can produce a more isolated wrist abduction movement.[2]
Wrist stabilization during gripping: Extensor carpi radialis brevis also participates in stabilizing the wrist during gripping and fist clenching. When the long finger flexors contract to close the hand, the wrist extensors work in synergy to hold the wrist in a slightly extended position, preventing it from collapsing into flexion.[1,2] Extensor carpi radialis longus is reported to be more active than extensor carpi radialis brevis during tight finger flexion, so extensor carpi radialis brevis should be seen as a contributor to this synergy rather than the sole dominant stabilizer.[2]


Innervation
Extensor carpi radialis brevis is supplied by the radial nerve, with contributions from the C7 and C8 nerve roots.[1,2] The motor branch to this muscle arises proximal to the arcade of Frohse, the fibrous superior border of the supinator muscle through which the posterior interosseous nerve passes.[1] The exact source branch, however, is variable. The branch may arise from the main trunk of the radial nerve, from the posterior interosseous nerve (the deep branch of the radial nerve), or from the point of bifurcation between them.[1]
Multiple cadaveric dissection studies have documented this variation in detail. Branovacki et al. (1998) found that in 60 paired forearms, the branch to extensor carpi radialis brevis arose from the posterior interosseous nerve in 45% of specimens, from the superficial branch of the radial nerve in 25%, and at the level of the radial nerve bifurcation in 30%.[6] Pérez-Mérida et al. (2018) reported a similar spread in 30 upper limbs, with origin from the deep branch in 50%, from the radial nerve trunk in approximately 27%, and from the superficial branch in approximately 23%.[7] Cricenti et al. (1994) also reported that the most common pattern was a single origin from the posterior interosseous nerve at the level of the humeroradial joint.[8]

Blood Supply
The arterial supply to extensor carpi radialis brevis comes from multiple sources. The principal blood supply is provided by a branch of the radial recurrent artery and by a branch of the radial artery that arises approximately one-third of the way down the forearm.[1] An additional proximal contribution comes from the radial collateral branch of the profunda brachii artery, which supplies the upper portion of the muscle near the elbow.[1] The radial recurrent arterial branches lie in close proximity to the deep branch of the radial nerve in the region between extensor carpi radialis brevis and the supinator muscle.[9]
